The CARI guidelines. Natural history and progression of atherosclerotic renal vascular stenosis.

چکیده
• Atherosclerotic renovascular stenosis is a potentially progressive disease. • Risk factors for progressive stenosis and renal artery occlusion include: – uncontrolled systolic hypertension (>160 mmHg) – diabetes mellitus – high grade (>70%) ipsilateral and contralateral atherosclerotic renal vascular disease (ARVD), and – significant baseline proteinuria. • Risk factors for atrophy include: – systolic hypertension (>160 mmHg) – stenosis of more than 60%, and – decreased renal cortical blood flow. • Risk factors for decline in glomerular filtration rate (GFR) include: – abnormal baseline creatinine (>128 mmol/L) – bilateral ARVD or ARVD in solitary kidney. • Atherosclerotic renovascular stenosis is associated with high mortality and morbidity due to atherosclerosis elsewhere, particularly coronary artery disease.

a. At 5 years (median 34 months), correction of renal artery stenosis (RAS), by balloon angioplasty with or without stenting (no distal protection) has no beneficial effect on blood pressure (BP) compared with medical therapy and is associated with an adverse event rate of 10–25%. متن کاملEffect of pyridoxal phosphate on atherosclerosis and nephropathy progression in the atherosclerotic rats
Background and Objectives: Diabetes vascular complications are the principal cause of death in the world. Therefore, we investigated the effect of pyridoxal phosphate (PLP) on the formation of atheromatous plaque formation and renal function in the atherosclerotic rats. Materials and Methods: Male Wistar rats were randomly divided into 4 equal groups (ten rats in each group): normal, ath...

Abstract Background: there are serological evidences about association between Mycoplasma pneumonia and cardiovascular diseases. It has been suggested that this Microorganism may have an important role in the progression of atherosclerosis. The aim of this study is to determine the presence of mycoplasma genome in atherosclerotic plaques by PCR. Materials and methods: This study was done on fif...
متن کاملAtherosclerotic Vascular Disease Conference: Writing Group V: medical decision making and therapy.
Atherosclerosis is a systemic disease with manifestations in multiple vascular beds. In each regional circulation, clinical events result from progression of the atherosclerotic lesion, superimposed thrombosis, and other dynamic processes, including ulceration, plaque rupture, intramural hemorrhage, fragmentation, and embolism. Evaluation and management of patients with atherosclerosis must be ...
متن کاملConcomitant Celiac, Superior Mesenteric, Renal, and Aorto-bi-iliac Aneurysms
Visceral artery aneurysms are rarely reported. They can occur in the context of a variety of underlying diseases. These aneurysms may coexist with non-visceral arterial aneurysms. Here we describe an 80-year-old man with concomitant superior mesenteric, celiac, renal, and aorto-bi-iliac aneurysms. The patient was a hypertensive and hyperlipidemic old man with history of transient ischemic attack...
